2007-08 returning student-athletes' medical forms

Returning Student-Athletes' Medical Forms
EACH YEAR, returning student-atheletes
are required to meet with a member of the athletic training staff on their
team's designated date and time, and go over their medical history from the
past year and determine their participation status.
Prior to this meeting, athletes must fill
out the forms listed below. All forms must be on file in the athletic training
department prior to the athlete's participation in any team practice/activity.
They can be printed out and mailed to the address listed below the links.
Simply mailing in the forms does not
exclude any athlete from meeting with a member of the athletic training staff.
Athletes will not be allowed to participate in any team practice/activity,
until a member of the athletic training staff has cleared them to do so.
