Designated Panel of Physicians
EMPLOYEE:
If you suffer a work-related injury, please follow this procedure:
-
Notify your supervisor immediately.
-
Seek medical treatment with one of the health care providers listed on the designated panel of physicians. (Please note that the Gannon Health Center should not be used as a treatment facility for work-related injuries.)
-
File a claim report with the Human Resources Department as soon as possible.
If faced with an extreme medical emergency, you may secure immediate assistance from a hospital, physician, or other health care provider of your choice. Subsequent treatment during the first 90 days from the date of this first visit, however, must be obtained from a physician or other health care provider listed on Gannon’s Designated Panel of Physicians.
-
Contact the Human Resources Department (in person or by phone) to report the injury and complete the appropriate paperwork. Human Resources will file the claim and contact you with a claim number which applies to all medical care and billing pertaining to your injury.
