Davis Vision - The Eyecare Advantage™     Benefits Summary Chart (Effective April 1, 2007)

Effective April 1, 2007, Davis Vision will serve as the new network provider for vision care.  This plan is part of Gannon's HighMark Blue Cross Blue Shield health insurance plan. The network of providers available through Davis Vision may be viewed by following the Highmark link below, scrolling down and clicking the Vision Benefit "GO" button.  Then simply follow the prompts.

MAIL-ORDER PRESCRIPTION DRUG PLAN 

Employees can save money by using the mail-order option for eligible maintenance prescriptions. The cost of a 90-day prescription is 66% lower by using the mail order option compared with filling three separate prescriptions at a local pharmacy. For more information about this plan, please contact the Human Resources Department.

The Blue Cross Blue Shield symbols on your PPO insurance ID card are recognized around the world; therefore, your coverage travels abroad offering important protection for you and/or your dependents while traveling.  Detailed information on the services provided are found in the Blue Cross Blue Shield Summary Plan Description in the section entitled The BlueCard Worldwide Program.  Additional information is available by contacting Member Services at (800) 241-5704.

UnumProvident Global Emergency Medical Services

Gannon's Long Term Disability carrier, UnumProvident, offers eligible colleagues worldwide emergency travel assistance services when traveling 100 miles or more from home, including travel abroad.  Detailed information about the services provided are found at www.unumprovident.com/travelassistance.  Contact the Human Resources Department to obtain a Universal Access Card for your travels.

FLEXIBLE SPENDING Summary Plan Description

Gannon's Flexible Spending Plan is administered by C. H. Reams and Associates.  This plan allows colleagues to apply pre-tax dollars to a medical reimbursement account and/or dependent care reimbursement account.  The plan year coincides with the health, vision, and dental plans fiscal year enrollment period, April 1 through March 30.  Eligible expenses for reimbursement may be viewed at Eligible Reimbursable Expenses.

LONG TERM DISABILITY:   Plan Document

 

Full-time employees are enrolled in UnumProvident's Long Term Disability Plan after a 91-day waiting period following their date-of-hire.  If, after six months of continuous disability, you are approved, the plan will pay 60% of your base wage (offset by social security).  Forms and additional information regarding eligibility are available in the Human Resources Department. 

 

 

LIFE INSURANCE:   UNUM Life Certificate     Uniform Premium Table

 

Gannon provides you with group life insurance equal to two times your annual salary, or $30,000, whichever is greater, effective the first of the month after a 30-day waiting period from your date of hire.  This coverage includes:

  • Waiver Coverage

  • Accidental Death & Dismemberment Coverage

  • Spouse Coverage of $10,000

  • Dependent Coverage or $6,000 per child
