Establishing Your File
Ä
Contact the Career Development and Employment Services Office to schedule an appointment to finalize your resume, complete a senior registration form and establish your file.

Provide copies of the documents listed below in the File Checklist as soon as the documents are available.

The CDES Office will keep your documents on file and forward them to prospective employers at your request for up to ten years.
It is your responsibility to keep the file current with copies of updated documents.
File Checklist
Students are to keep original documents; file should contain copies of the originals.

Current resume

Final evaluations of Student Teaching (provided by Education Department)

Unofficial copy of transcript*

Copy of Teaching Certificate or In Process Letter from Education Department

List of references (3-5 names, addresses and phone numbers)

Copy of Act 34 and Act 151 Clearances (optional)

Copy of Praxis Scores (optional)

Upon receipt of your final student teaching evaluations, our office will request an unofficial copy of your transcript from the Registrar’s Office after the
degree date conferred has been recorded.
An unofficial copy is usually all that is required.
However, if a school district requires an official transcript, you must request this personally from the Registrar’s Office as your signature is required.
Please notify the CDES Office if an official transcript is required; we will forward your credentials file to the Registrar’s Office to be mailed along with the transcript.
Requesting to have your file mailed to prospective employers

Review file contents in the CDES Office.
Be sure you’ve signed a Release of Information Form.
Ä
You are responsible for making the first contact with a prospective employer by forwarding a resume and cover letter.
When you receive a response and a request for further information, contact the CDES Office by phone, fax or mail to request your file be mailed to the employer.
Your file will be mailed within two to three days of your request.
Names and addresses of employers to whom we’ve forwarded your file will be kept in your folder.

If you are asked to return a completed application form which has been sent to you, a credentials file should accompany the application form.

You may be asked to assist with the copying of your file contents to expedite your request.

The Career Development and Employment Services Office will send out a maximum of fifteen credentials files at no charge.
After that, a processing fee of $2.00 (cash only) will be charged for each file mailed.
It is very important to update your Teaching Credentials File on a regular basis.
