What is Worksite Wellness
Worksite
Wellness is a clinically proven, data-driven health promotion program that aims
to provide you with the information and means needed to reduce your risk of
dangerous health conditions.
What is the Personal Wellness Profile?
The
first step in the wellness process is the completion of a 40-question Personal
Wellness Profile used to assess individual risk concerning
heart health, nutrition,
exercise, cancer and other health topics.
What is the Lifestyle Screening?
The lifestyle screening is performed
by Highmark professionals and is done to measure total cholesterol,
glucose, blood pressure and body mass index. (A blood-draw is required.)
What
is the Personal Wellness Plan?
The
Personal Wellness Plan is
a computer-generated plan created from the data obtained from the
Personal Wellness Profile and the lifestyle screening. This individualized plan
alerts participants to their degrees of risk concerning our
nation’s most common and serious illnesses,
and provides recommendations on what HealthPLACE programs and classes would most
effectively diminish their health risk.
How do I participate in wellness program?
The
first step in the wellness program process is the completion of a Personal
Wellness Profile. You will receive this questionnaire when you sign-up for the
screening event.

Will my doctor see the results of my lifestyle
screening?
The
results of your screening will be sent to your doctor only at your request. If
you do not wish to have the results sent to your doctor, they will remain
completely confidential.
Will my manager
see the results of my Personal Wellness Profile, lifestyle screening or Personal
Wellness Plan?
Management will never see the results of your
Personal Wellness Profile, lifestyle screening or Personal Wellness Plan.
Who will have access to the information obtained
during the lifestyle screening data?
All confidential
lifestyle screening information will be stored in a special database that can be
accessed by Highmark professionals. The data will be stored for a period of
seven years, after which point it will be destroyed.
