By submitting this form, you certify that you understand the intramural eligibility rules and have checked all the players on your team.
If there is any discrepancy, you will assume full responsibility. If there are any questions about rules or eligibility, you must contact the Activities Supervisor.
Your team will not be scheduled until the forfeit fee of $25 is paid.
Individual participants should be duly aware of the risks and hazards involved in intramural sports and should voluntarily select to utilize the Gannon facilities and participate in programs recognizing present conditions and further agree to voluntarily assume in all risks of loss, damage, or injury that may be sustained while using the Gannon facilities or participating in programs.
Gannon Intramural Department assumes no responsibility for costs involved with individual injury or property damage incurred in connection with the use of university facilities. Individuals are reminded that they should review their own personal circumstances to determine if they have adequate insurance or protection in case of injury resulting from the use of university facilities or participation in programs. It is recommended that all participants have a complete physical before engaging in any physical recreation programs.
